Endorsing the Medical Screening Questionnaire (MSQ)

What you need to know

  • As part of the medical screening for pre-enlistees, your son or ward needs to provide his medical information, including medical history and childhood illness, on the Medical Screening Questionnaire (MSQ).
  • He should provide detailed medical information, especially if it might affect his ability to do certain tasks. This ensures he is assigned an appropriate Physical Employment Standard (PES) grade.
  • Your son or ward must nominate one parent or guardian who knows his medical history well to endorse the MSQ.

What the Endorser needs to do

  • Log in to the OneNS web portal to view the MSQ.
  • Verify that the information that your son or ward submitted in his MSQ is correct and complete.
  • Endorse the MSQ before your son or ward‘s pre-enlistment medical screening appointment at the Central Manpower Base (CMPB).

Importance of endorsing the MSQ

The MSQ will include questions related to family medical history, drug allergies, and more. Filling up this form accurately is important for medical officers to note any issues from current or pre-existing medical conditions.

Your son or ward must nominate one parent or guardian with knowledge of his medical history to verify and endorse the MSQ. It‘s crucial that the endorser carefully looks through your son or ward‘s MSQ to ensure that any issues that might affect his ability to perform certain duties are noted, so that he is assigned an appropriate PES grade.

If his MSQ is not endorsed before his medical appointment at CMPB, his PES grading may be delayed. This might lead to delays in his enlistment and any subsequent university matriculation after NS.

How to endorse MSQ

The parent or guardian nominated to endorse your son or ward‘s MSQ will receive an SMS and email from MINDEF that will request for endorsement of the MSQ. They:

  • will need Singpass to log in to the OneNS web portal through the website.
  • must review the MSQ and check that your son or ward did not miss out on any important information, like family medical history and childhood illness.
  • can provide comments or additional information on your son or ward‘s medical history, if need be.
  • have to verify that the information provided is correct and proceed to endorse.
