What you need to know

  • If your son or ward is assigned Physical Employment Standard (PES) D, this means more medical information is needed before his PES can be determined.
  • He will need to provide more medical documents or attend medical reviews, or both, with specialists, to determine his medical fitness for National Service (NS).
  • Attending a scheduled medical appointment is compulsory. He should avoid rescheduling his medical appointment as doing so may delay his enlistment and any subsequent university matriculation post-NS.
  • The Further Reporting Order (FRO) sent by MINDEF, that your son or ward can also find on the OneNS web portal, will include the details of the document requirements and appointment information.
  • Not complying with the FRO is an offence under the Enlistment Act, and he may be fined up to $10,000, jailed for up to 3 years, or both.

What you need to do

  • Review the FRO with your son or ward to ensure that he complies with the deadlines and appointment information within.
  • Check your son or ward’s FRO if you need to be present at his specialist appointments, as it may be required.
  • Ensure your son or ward brings the necessary documents, such as a printed copy of Letter of Identity (LOI), for his further medical review appointments.
  • Ensure your son or ward uploads his Progress Report (PR) on Medical Status on the OneNS web portal after he attends his appointment at public hospitals if he has been instructed to do so in his FRO.

Medical review process

Your son or ward will be informed if he needs to attend medical reviews. The Further Reporting Order (FRO) sent by MINDEF, that he can also find on the OneNS web portal, will include details of the required documents and appointment information. If further medical reviews are needed, he will receive a new FRO via mail, which will also be available on the OneNS web portal. The medical review process for your son or ward may be one or more of these scenarios, depending on his medical condition:

Scenario 1 – Appointment at the National Heart Centre Singapore (NHCS)

  • Your son or ward must attend follow-up appointments with NHCS.
  • He must bring along the FRO and referral letter sent to him or print out a copy from the OneNS web portal.

Scenario 2 – Appointments at National University Hospital (NUH), Raffles Hospital (RH) and Singapore General Hospital (SGH)

  • Your son or ward must attend follow-up appointments with the hospitals.
  • He must bring along the FRO, Letter of Identity (LOI) and referral letter sent to him or download a copy from the OneNS web portal.

Scenario 3 – Appointments with other public hospitals

  • Your son or ward must attend follow-up appointments with other public hospitals.
  • He must bring along the FRO, LOI, Progress Report (PR) on Medical Status, and referral letter sent to him or print a copy from the OneNS web portal.
  • Your son or ward has to ensure that the PR must be completed by the attending doctor.
  • Submit his appointment card and PR on the OneNS web portal if he has been instructed to do so in his FRO.

Scenario 4 – Appointment at the Medical Classification Centre (MCC) in the Central Manpower Base (CMPB)

  • Your son or ward needs to report to the MCC for follow-up tests or reviews, or both.
  • The details and preparation instructions will be explained in the FRO sent by MINDEF, that your son or ward can also find on the OneNS web portal.
  • Your son or ward must attend the appointment.

Scenario 5 – Submit more medical documents

  • The Medical Officer (MO) will need more medical proof or medical history documents from your son or ward.
  • The documents required and other details such as deadline will be explained within the FRO sent by MINDEF, that your son or ward can also find on the OneNS web portal .
  • Your son or ward has to submit the required documents on the OneNS web portal by the deadline and attend any follow-up medical appointments.
  • If your son or ward is unable to obtain the required documents by the deadline, he may extend the submission deadline on the OneNS web portal.

After your son or ward has completed all his medical reviews, no further action is required from him. He can check his Physical Employment Standard (PES) on the OneNS web portal after two months.

Changing his medical appointment

If your son or ward is unable to attend his medical review appointments on the scheduled date, he may reschedule his appointment. To do so, he should follow the instructions in the table below, which varies according to the hospital and speciality:

For the following hospitals, national specialty centres, or medical centres:

  • Medical Classification Centre or Military Medicine Institute

  • National University Hospital

  • Raffles Hospital

  • Singapore General Hospital

  • Changi General Hospital (Respiratory Medicine)

  • Institute of Mental Health

  • Khoo Teck Puat Hospital (for 2D-Echo)

  • Sengkang General Hospital (Psychology and Respiratory Medicine)

  • Tan Tock Seng Hospital (Endocrinology and Renal Medicine)

  1. Fill in the form to reschedule your appointment and provide 2 preferred dates. He will need to submit supporting documents (such as Medical Certificate or letter from his school).
  2. After the appointment is rescheduled, he will receive an SMS and a new set of documents via mail, which will also be available on the OneNS web portal.

  1. Call or email the hospital, national specialty centre or medical centre to reschedule his appointment.
  2. Update his appointment details on the OneNS web portal.
  3. Download and print out the new Further Reporting Order (FRO) and Letter of Identity (LOI) from the OneNS web portal.
  4. Bring the new FRO and LOI, along with his original Progress Report (PR) on Medical Status and referral letter to the appointment.

Letter of Identity (LOI)

The Letter of Identity (LOI) waives consultation and investigative fees up to diagnosis at public hospitals. It does not cover the cost of treatment or medication provided after diagnosis.

Your son or ward should follow the instructions in his FRO and bring his LOI for appointments. If not, he may not be allowed to attend the appointment or may have to pay for the full medical fees.

He may need to request for a new LOI on OneNS web portal if:

  • He’s going for a subsequent appointment at a public hospital.
  • He has changed his appointment date.

Progress Report on medical status

The Progress Report (PR) is a form that he must ask the attending doctor at the hospital to complete after every appointment. This is an important document for the Medical Officer to determine your son or ward’s medical fitness for National Service (NS). The details on the PR will also determine if your son or ward needs a subsequent appointment or new Letter of Identity.

Your son or ward must submit his PR through the OneNS web portal after his appointment if he has been instructed to do so in his Further Reporting Order.

He needs to keep all forms issued to him by the Medical Classification Centre and a hard copy of his PR for at least 6 months, as he may be asked to provide them if necessary.

Requesting for a medical review

Changes in medical condition

Your son or ward must send a doctor’s memo to the Medical Classification Centre (MCC) via email at contact@ns.sg. This is usually the case if:

  • He developed new injuries or medical conditions only after his medical screening.
  • He is scheduled for surgery and may not recover in time.

If he needs further reviews by the Medical Officer, MCC will inform him through the OneNS web portal. This may lead to a delay in his enlistment date and any subsequent university matriculation post-NS.

PES BP but lost weight

If your son or ward has lost a lot of weight between his medical screening and enlistment date, he can go to MCC at the Central Manpower Base (CMPB) for a medical review. He can only do so on weekdays from 2pm to 4pm.

Other reasons

If there are other reasons that might affect your son or ward’s PES (such as going for LASIK or refractive surgery), he will need to submit the relevant supporting documents by:
