What you need to know

  • If you are assigned Physical Employment Standard (PES) D, more medical information is needed before your PES can be determined.
  • You will need to either provide more medical documents, attend medical reviews, or both, with specialists to determine your medical fitness for National Service (NS).
  • Attending a scheduled medical appointment is compulsory. Avoid rescheduling your medical appointment as doing so may delay your enlistment and any subsequent university matriculation after NS.
  • The Further Reporting Order (FRO) sent by MINDEF will include the details of the document requirements and appointment information. You can also find this on the OneNS web portal.
  • Not complying with the FRO is an offence under the Enlistment Act, and you may be fined up to $10,000, jailed for up to 3 years, or both.

What you need to do

  • Read the FRO carefully and ensure that you comply with the instructions, deadlines, and appointment information.
  • Check the FRO if your parent or guardian must attend the specialist appointments, as it may be required.
  • Ensure you bring the necessary documents, such as a printed copy of Letter of Identity (LOI), a letter that waives consultation and investigative fees up to diagnosis at public hospitals, for your medical review appointments.
  • Ensure you upload your Progress Report (PR) on Medical Status, which is a form that you must ask the attending doctor at the hospital to complete, on the OneNS web portal after you attend your appointment at public hospitals, if you were instructed to do so in the FRO.

Medical review process

You will be informed if you need to attend medical reviews. The Further Reporting Order (FRO) will include details of the required documents and appointment information. The FRO is sent by MINDEF, and you can find it on the OneNS web portal as well. Your medical review process may be 1 or more of these scenarios, depending on your medical condition:

Scenario 1 – Appointment at the National Heart Centre Singapore (NHCS)

  • You must attend follow-up appointments with NHCS.
  • You must bring along the FRO and referral letter sent to you or print out a copy from the OneNS web portal.

Scenario 2 – Appointments at National University Hospital (NUH), Raffles Hospital (RH) and Singapore General Hospital (SGH)

  • You must attend follow-up appointments with the hospitals.
  • You must bring along the FRO, Letter of Identity (LOI) and referral letter sent to you or download a copy from the OneNS web portal.

Scenario 3 – Appointments with other public hospitals

  • You must attend follow-up appointments with other public hospitals.
  • You must bring along the FRO, LOI, Progress Report (PR) and referral letter sent to you or print a copy from the OneNS web portal.
  • You have to ensure that the PR must be completed by the attending doctor.
  • Submit your appointment card and PR on the OneNS web portal if you has been instructed to do so in your FRO.

Scenario 4 – Appointment at the Medical Classification Centre (MCC) in the Central Manpower Base (CMPB)

  • You need to report to the MCC either for follow-up tests, or reviews, or both.
  • The details and preparation instructions will be explained in the FRO sent by MINDEF, that you can also find on the OneNS web portal.
  • You must attend the appointment.

Scenario 5 – Submit more medical documents

  • The Medical Officer will need more medical proof or medical history documents from you.
  • The documents required and other details such as deadline will be explained within the FRO sent by MINDEF, that you can also find on the OneNS web portal.
  • You have to submit the required documents on the OneNS web portal by the deadline and attend any follow-up medical appointments.
  • If you are unable to obtain the required documents by the deadline, you may extend the submission deadline on the OneNS web portal

After you have completed all your medical reviews, no further action is required from you. You can check your Physical Employment Standard (PES) on the OneNS web portal after 2 months. 

Changing your medical review appointment

If you are unable to attend your medical review appointments on the scheduled date, you may reschedule your appointment. To do so, you should follow the instructions in the table below, which varies according to the hospital and speciality:

For the following hospitals, national specialty centres, or medical centres:

  • Medical Classification Centre or Military Medicine Institute

  • National University Hospital

  • Raffles Hospital

  • Singapore General Hospital

  • Changi General Hospital (Respiratory Medicine)

  • Institute of Mental Health

  • Khoo Teck Puat Hospital (for 2D-Echo)

  • Sengkang General Hospital (Psychology and Respiratory Medicine)

  • Tan Tock Seng Hospital (Endocrinology and Renal Medicine)

  1. Fill in the form to reschedule your appointment and provide 2 preferred dates. You will need to submit supporting documents (such as a Medical Certificate or letter from your school).
  2. After the appointment is rescheduled, you will receive an SMS and a new set of documents via mail, which will also be available on the OneNS web portal.

  1. Call or email the hospital, national specialty centre or medical centre to reschedule your appointment.
  2. Update your appointment details on the OneNS web portal.
  3. Download and print out the new Further Reporting Order (FRO) and Letter of Identity (LOI) from the OneNS web portal.
  4. Bring the new FRO, LOI, your original Progress Report (PR) on Medical Status, and referral letter to the appointment.

Letter of Identity (LOI)

The Letter of Identity (LOI) waives consultation and investigative fees up to diagnosis at public hospitals. It does not cover the cost of treatment or medication provided after diagnosis.

Follow the instructions in your FRO and bring your LOI for appointments. If not, you may not be allowed to attend the appointment or may have to pay for the full medical fees.

You may need to request for a new LOI on OneNS web portal if:

  • You are going for a subsequent appointment at a public hospital.
  • You has changed your appointment date.

Progress Report on medical status

The Progress Report (PR) is a form that you must ask the attending doctor at the hospital to complete after every appointment. This is an important document for the Medical Officer to determine your medical fitness for National Service (NS). The details on the PR will also determine if you need a subsequent appointment or new Letter of Identity.

You must submit your PR through the OneNS web portal after your appointment, if you have been instructed to do so in your Further Reporting Order.

You need to keep all forms issued to you by the Medical Classification Centre and a hard copy of your PR for at least 6 months, as you may be asked to provide them if necessary.

Requesting for a medical review

Changes in medical condition

You must submit a doctor’s memo to the Medical Classification Centre (MCC) on the OneNS web portal under Manage Medical Matters. This is usually the case if:

  • You developed new injuries or medical conditions only after your medical screening.
  • You are scheduled for surgery and may not recover in time.

If you need further reviews by the Medical Officer (MO), MCC will inform you through the OneNS web portal. This may lead to a delay in your enlistment date and any subsequent university matriculation after NS.

PES BP but lost weight

If you have lost a lot of weight between your medical screening and enlistment date, you can go to MCC at the Central Manpower Base (CMPB) for a medical review. You can only do so on weekdays from 2pm to 4pm.

Other reasons

If there are other reasons that might affect your PES (such as going for LASIK or refractive surgery), you must submit the relevant supporting documents by:
